By signing up for the RC4Kids or SportsCamps4Kids (referred to as "The Sports Workshop"), you agree to the following:

Parent/Caregiver Consent: I agree with my child (whose name is on this form) joining The Sports Workshop.

Safety Info: I will inform The Sports Workshop team of any conditions that could affect my child’s safety or activities they should avoid.

Risk Acknowledgment: I understand that some activities might involve risks. The Sports Workshop will strive to ensure safety but is not responsible for injuries or lost items.

Medical Treatment: I authorise The Sports Workshop staff to seek medical help for my child if necessary, at my expense.

Behaviour Policy: If my child does not follow the rules or misbehaves, I will be contacted to pick them up.

Sign In/Out: I will sign my child in and out every day.

Pickup Authorisation: If someone else will pick up my child, I will notify The Sports Workshop staff on that day.

Activity Changes: The Sports Workshop may change or substitute activities as needed.

Photo/Video Permission: I consent to photos and videos of my child being taken during the workshop for marketing purposes.

Refunds:

• A 50% refund of the camp fee will be given up to 2 weeks before the camp start day.

• No refunds are available within 1 week of the camp start day.

• Sibling Discount: A sibling discount will be deducted from the refund amount if applicable.

• A non-refundable processing fee of $15 will apply to all refunds.

Marketing Consent: By registering, you consent to receive marketing communications from us. You can opt-out at any time by following the unsubscribe instructions in our emails and/or messages.

Policy Changes: We may update this policy.
